Sometimes, my sense of humor completely misses the mark.
I have a theory that a lot of people who do or say creepy things are actually trying to be friendly. Their problem is that what passes as "friendly" between actual friends is a lot different than what another person might find to be "friendly." In your example, the dude who be like 'wake SO with blowjob ha ha' might actually think that he's trying to be "casual" in a very "bro-tastic" way. In several other examples presented, I just don't have any excuse for them.
Let's take a more concrete example: are we friends? On many levels, I'm not entirely sure. We've chatted via PM, we've yik-yaked on games, and I'm pretty sure you know more about me than many people here. I can say with some certainty that if we ever had the pleasure of actually meeting (and thus violate my Prime Directive) we would probably have drinks and you'd find me to be a borderline narcissist alcoholic. (As opposed to lawyer, which may be synonymous.)
But, let's suppose we're friends, yeah? We met through games. I'm guessing at first you didn't care who I was or what I was about, and the feeling was mutual. Over the years of casual acquaintance and occasional argument, I've built a healthy respect for you and what you do and what you have to say. And when you've needed a hand or something, I think I was respectful with how I stepped in to do something. So we started with indifference and, over time, built it into what it is now.
Going back to your example, the fellow that suggested you give your SO a fellatio alarm clock just rushed into "bro-tastic mode" rather than, as I said, starting with indifference and building from there.
I've found in my (now years of) karaoking experience that the friends I've found (and now care deeply about) started off as singers. You appreciate them, they appreciate you. But did we give a fuck about each other (really) immediately? I don't think so: I had no agenda, no plans, nothing, so shit built up over time. I started with indifference.
I also have enough self-respect to hide my lack of human decency by treating other people politely. Much like a Spina, though, being polite does not always mean being nice.
Yeah, I agree with this.
A lot of what comes off as lackluster social skills is basically just not wanting to put in the work (or thinking you can skip over the work) of getting to the point where your normal social behavior is acceptable by the person you're interacting with. Translates to RP, too, and what you're willing to play with some people and not with others.
Does not, under any circumstances, excuse it, though.

I'm playing a Firefly ship captain. My character's goal is to keep the ship flying. I do lots of RP about this. Some portion of that RP is supported by coded systems, whether it's a haggle die roll to get a good deal, or a coded econ system where I have to spend some measure of my playtime running cargo from point A to point B. Is that an OOC game? I, as a player, don't give a crap about virtual cargo or imaginary money. I'm just abiding by the game's framework that lets me tell stories about them.
Combat is another instance. My poses are literally influenced by what the combat system says - was my character shot in the leg? OK, RP that. Did they just blow somebody's head off? OK, RP that. Bob is getting piled on by Cylons? OK, RP going to help him. These just really don't feel like OOC games to me. They're just as much a part of the roleplaying experience as the rules and dice in a TTRPG.
You're deviating from what spurred this topic of conversation into an aergument that's pointless because we agree, you're just interpreting my words as meaning something they don't. I am specifically referring to the type of distinction Ominous made here:

I think these desires are mutually exclusive. Plotting and intrigue needs something concrete to plot and intrigue over and that concreteness comes from game mechanics. Otherwise you're just stabbing each other in the back over the color of the draperies. I mean, I guess you could plot an assassination on your brother, so you can inherit a pointless crown that doesn't mean anything, but that strikes me as just being mean to the player of the brother for no reason, because, again, the crown is pointless. Kind of like kicking a dog, because it was sleeping and no one was watching, so you could get away with it.
I do not need the crown (or your spaceship) to be backed by a system mechanic in order to roleplay either being important, because to me, narrativity is what's important and the rest props it up.

Many MU players are happy to dip in and out of narrative/system as their mood suits them.

It's not an absolute thing, but rather a fluid thing we can slip in and out of, liek you said, often without even realizing.
The difference is: you can have a game that is purely these "OOC" things (tons of video games, etc, like @L-B-Heuschkel mentioned above) and you can have a game that is purely narrative, without any code beyond something to type in (in the case of online). The difference, IMO, is that the former is not actually roleplaying (despite, again like @L-B-Heuschkel said, the highjacking of the term), and the latter is.
I mean, we can play semantic games all day (not saying we are, just saying we could) about what the word "roleplaying" really means, but that's what it means to me: narrativity, if the characters and story are the point, those two things are enough to make it roleplay (whether you enjoy that barebones type of play or not notwithstanding). If you don't have them, it's not. It's something else that has, for some reason, been conflated into the same term.
I like Dorothy Parker a lot.
I like Percy Shelley's "Love's Philosophy" a lot.
I like writing off-the-cuff limericks, as a lot of people on Spirit Lake found out one very weird morning.
I also sometimes write poetry IC because whatever is happening strikes a particular character, and that's my favorite thing.
I think Hemingway's poetry is Vogon poetry without the good bits.

A friend and I used to troll people on a comic book game we ran because we talked so much off-game that a lot of people just assumed we lived together. Like people would ask "where's Coin?" and she'd be like, "he just walked into the house, give him a second, geez". We had fun trolling people, some of the others on the game got into it too. Basically, anyone in on the joke with a sense of humor would help us troll other people.

Nothing matters if the players don't care about it. Mechanics, system, all that is there to facilitate or guide the narrative. Some people may only want to play a certain narrative because it uses a certain system or code or whatever, but that doesn't mean that the real value isn't on the narrative. If people don't care about the story, they aren't going to play it, no matter how cool the system is.
Disagree. I've been on games that were basically treated like MUDs, where the only thing people gave a shit about was the system. Interesting character? Not necessary. Don't even pose until it's time to roll and kill things and get xp!
Keep reading my further posts and you'll see that I deem that not really RP so much as an OOC game. I mean, it exists, but they are two fundamentally different things that have, for some reason, been conflated in some contexts, which is dumb.
