I don't know that there's an ideal for me, depends on if the scene is plot-related or TS for TS's sake. In the former case, it's usually the logical followup of a scene, two characters going from one thing (sparring, talking about plot, etc) to another (sexy times). In the latter case, the foreplay usually starts within the first couple of poses.
Also bonus question! Do you ever pause in the middle of TS then pick it up at a later time?
Yes, although I usually prefer to pause between foreplay and intercourse, or between bouts, rather than in the midst of intercourse. It's not particularly easy (for me) to pick up a scene again in the midst of bumping uglies and get right back down to bumping them together.

I totally get wanting to not leave the after hanging. My personal favorite way to do it is to handwave the actual sex, but still to play out the after-glow. It's not as explicit, but I still know where their relationship stands after they've had sex.

@silverfox I frequently RP some foreplay, do a quick fast forward, RP some afterwards. It's like the "highlights" or something.

I've started scenes that were understood to be pretty much specifically for TS purposes, more or less, and then had hardly any sex happen because it gets too late on account of all the scene setting, so that's a thing. But typically they just happen at the end of romantic or flirty or just awkwardly sexually tense RP.
Sometimes the RP leading up to it is completely chaste, but there's always just some kind of IC segue that makes sense, it's not like <OOC> Okay now that we've wrapped up the minutes of the wizard gardening association, let's get boning.
Also bonus question! Do you ever pause in the middle of TS then pick it up at a later time?
Sometimes more than once. As in the same scene happens over three different sessions, at least. Usually when there's some kind of emotional or sexual development going on, I guess? Like, oh... let's not fade to black there, I want to see how they react to this.

How much do your TS habits change based on the MU*'s themes? That is, do you find your characters are more promiscuous on say, a Kushiel game than a World of Darkness one? Do you TS considerably more often? Is it easier (or harder) to find partners - and are they actually better at TS on average?

What is the definition of a SAFE TS partner, and if TS is just creative writing why is safety an important factor?

@Arkandel I think there's a slight mismatch in your first question. Character promiscuity and TS habits aren't always linked. Promiscuous characters don't need to engage in TS at all, and one person could play several monogamous characters.

A person that sees TS in the same light as you do. The fact that we've had several reasonably reliable reports of stalkers, crossing games and crossing into RL, indicates that just because X-person believes that TS is only creative writing, Y-person isn't guaranteed to share the same opinion.

Entirely. A WoD game allows for different play than a BSG game, for example.

There's a reason I have people I trust to rp relationships with. I know (a) the RP will be good and (b) things like OOC boundaries will be trusted.

My TS habits factored more on game selection than anything, and were more baseline the same regardless of game. So I tended to avoid games like Kushiel, Shang, etc because I wasnt looking to write that kind of stuff (best to mitigate by avoidance). I'm weird, though, because I think the more in my brain I call it TS the more something other than writing a scene it becomes. So I never really made purposefully promiscuous characters with the intent to TS about it.
As far as the "SAFE" question goes, I'm in a similar boat with @Tinuviel and @Ganymede. SAFE as a term only had to do with the age and "ooc bleed factor" of the player. I was less concerned about stuff like "whether their character would dump mine" or anything clingy. I just wanted to make sure that I wasn't writing anything inappropriate with a minor or that it wouldn't adversely affect some less stable RL person on the other end of the keyboard.
